W grze Monster Girls and the Mysterious Adventure 2 napędza rozgrywka w stylu turn-based roguelike RPG, w której gracze przemierzają siatkowe mapy w lochach. Tutorial i fabuła wprowadzają nowicjuszy w ten tajemniczy świat.
System przyjaźni pozwala zaprzyjaźniać się z unikalnymi potworami - zabieramy ich teraz trzech do lochów (z sześcioma w rezerwie) i wymieniamy w trakcie przygody.
