Ghost War provides competitive player-versus-player matches in a 4v4 format. Available variants include Elimination, where teams aim to remove all opponents within a time limit, Extraction, focused on rescuing or protecting a hostage, and Sabotage, centered on capturing key objectives.
Ghost Mode adds a separate hardcore layer to the campaign experience. It restricts players to a single primary weapon, introduces permadeath, removes AI teammates, and enforces realistic reloading rules where unused ammunition in magazines is lost. This mode can be played solo or with others.

Bolivia serves as the primary setting, rendered with varied terrain that encourages exploration both on established roads and through off-road paths. The central conflict pits players against the Santa Blanca cartel, whose influence extends across the map through bases, convoys, and patrols. Progression involves gathering intelligence and executing operations that gradually erode the cartel's hold on different provinces.
